The first step
How to claim
The exact requirements depend on the policy and insured event. These steps reflect the current funeral-claim forms supplied by Westlife.

Notify the claims team
Call +267 3710619 or email lifeclaims@westlife.co.bw as soon as possible. We will confirm which forms and documents apply.

Complete the forms
For a funeral claim, complete the Funeral Claims Form in black pen and block letters. The claimant must also complete the individual KYC form.
- 03
Prepare supporting documents
Prepare the claimant's identification, a certified death certificate, banking details, and a police report where the cause of death was unnatural. Additional documents may be requested.

Submit and stay in contact
Submit the completed forms and supporting documents to the claims team. We will assess the claim and contact you if anything else is required.
